Vitrectomy Combined with Scleral Buckling in Patients with Inferior Retinal Breaks

چکیده
Eyes with minimal PVR and anteriorly located break(s) can be successfully managed by pneumatic retinopexy, scleral buckle or vitrectomy while eyes with posterior break(s) or significant PVR need vitrectomy along with tamponading gas or oil2. The specific gravities of most of the internal tamponading agents are less than balanced saline solution. That is why oil or gas bubble floats at the top most position, pressing the retina and providing a tamponade for superior retina. However, its effect on the inferior retina is not enough to press the retina down to pigment epithelium layer and it fails to provide a tamponade3. This poses a problem in managing patients having high grade PVR and inferior breaks, since neither scleral buckle nor vitrectomy alone can keep the retina attached.

Pars Plana Vitrectomy versus Combined Scleral Buckling—Pars Plana Vitrectomy for Phakic Rhegmatogenous Retinal Detachment with Inferior Breaks
Aims: To compare the results of pars plana vitrectomy (PPV) and combined scleral buckling—PPV (SB/PPV) in phakic rhegmatogenous retinal detachments with inferior breaks. Methods: Randomized, prospective, clinical controlled trial of forty consecutive phakic eyes with primary rhegmatogenous retinal detachment, associated with inferior breaks and not complicated by proliferative vitreoretinopathy...
متن کاملSCIENTIFIC REPORT Management of primary rhegmatogenous retinal detachment with inferior breaks
Background: Rhegmatogenous retinal detachments (RRD) with inferior breaks are usually treated by scleral buckling (SB) or pars plana vitrectomy (PPV) or a combination of both methods. However, applying a SB during PPV may produce a risk of choroidal haemorrhage. Following a recent pilot study showing that such cases can be safely treated by PPV without SB the authors re-examined their managemen...
متن کاملComparison of scleral buckling and vitrectomy for retinal detachment resulting from flap tears in superior quadrants.
PURPOSE To compare the surgical results of vitrectomy and scleral buckling for uncomplicated superior retinal detachment caused by flap tears. METHODS Included in the study were 225 patients (225 phakic eyes) undergoing primary surgery by three surgeons between January 1990 and December 1996 for superior retinal detachment caused by flap tears (138 eyes by scleral buckling, 87 eyes by vitrect...

come clinically visible, resulting in the blue appearance of the sclera. We report the treatment of rhegmatogenous retinal detachment in 4 eyes of 3 patients with OI. Scleral buckling surgery, pneumatic retinopexy, and primary vitrectomy are the standards for repair of uncomplicated rhegmatogenous retinal detachments. In patients with OI, however, the thin sclera provides an increased risk of s...
